Patient Navigator/Data Analyst

Physician Affiliate Group of New York (PAGNY) is a prominent physician group linked with NYC Health + Hospitals, providing services across public hospitals and community practices. This Patient Navigator/Data Analyst role exists to support a CDC-funded initiative aimed at controlling HIV and communicable diseases, requiring collaboration with healthcare professionals.

In this role you can expect to have the responsibilities:

• Identify candidates for HIV testing in the emergency department.
• Conduct and document rapid HIV tests, ensuring compliance with hospital guidelines.
• Link newly diagnosed HIV+ patients to care at designated clinics.
• Administer counseling and education for PrEP/PEP for HIV- clients.
• Maintain detailed documentation of patient interactions in the EMR.
• Engage in quality improvement and program enhancement interviews.
• Prepare and manage databases, performing statistical analyses.
• Generate research summaries, tables, and reports.

This role requires you to have:

• Bachelor's Degree in relevant field required; Master's preferred.
