In this role you can expect to have the responsibilities:

• Coordinate care and direct services for mental health consumers.
• Work with clients, families, and community resources to secure necessary services.
• Focus on the Transitions of Care (TOC) segment of the Tennessee Health Link program.
• Conduct data gathering and analysis for the TOC program.

• Bachelor's degree in a related field or equivalent credit hours.
• Self-motivated with excellent time management.
